Ipsilateral sciatica on femoral nerve stretch test is pathognomonic of an L4/5 disc protrusion.
نویسنده
چکیده
The straight leg raising test and the femoral nerve stretch test exert traction on the sciatic and femoral nerve, and the lumbosacral plexus and roots. In 40 patients with a suspected L4/5 disc protrusion, ipsilateral sciatica was induced by the femoral nerve stretch test. We believe that this is a pathognomonic sign of a lateral protrusion at L4/5 level.

Mixter and Barr in 1934 first described the herniated disc to be a cause of segmental leg pain (sciatica or femoralgia). Acute low back pain is a relatively common condition and is accompanied by sciatica in only 1–2% of cases. Patients presenting with acute low back pain alone are therefore unlikely to have a disc prolapse. Lumbar intervertebral disc prolapse is most prevalent between the ages...
متن کاملThe Furcal Nerve Revisited
Atypical sciatica and discrepancy between clinical presentation and imaging findings is a dilemma for treating surgeon in management of lumbar disc herniation. It also constitutes ground for failed back surgery and potential litigations thereof. Furcal nerve (Furcal = forked) is an independent nerve with its own ventral and dorsal branches (rootlets) and forms a link nerve that connects lumbar ...
متن کاملEffect of ligamenta flava hypertrophy on lumbar disc herniation with contralateral symptoms and signs: a clinical and morphometric study
INTRODUCTION The purpose of this study was to determine whether ligamentum flavum hypertrophy among disc herniated patients causes contralateral pain symptoms. For this reason we measured the thickness of the ligament in disc herniated patients with ipsilateral or contralateral symptoms. MATERIAL AND METHODS Two hundred disc herniated patients with ipsilateral symptoms as group I were compare...
متن کاملThe Efficacy of Infliximab in Sciatica Induced by Disc Herniations Located at L3/4 or L4/5: A Small-Scale Randomized Controlled Trial
Objective: To evaluate the efficacy of Infliximab, a monoclonal antibody against tumor necrosis factor alpha (TNF ), in patients with acute/subacute sciatica secondary to herniated disc at L3/4 or L4/5. Design: A randomized controlled trial. Background: The results from our randomized trial demonstrated no efficacy for Infliximab but the subgroup results suggested that Infliximab may be effecti...

BACKGROUND The transpsoas lateral surgical approach has been advocated as an alternative to direct anterior approaches for less invasive or minimally invasive access to the spine. Postoperative thigh pain, paresthesia, and/or weakness have been described after the use of this surgical approach. The purpose of this cadaveric anatomic study is to provide a description of the lumbar plexus as it r...
